Our Financial Policy


 

Vascular Surgery Associates believes that open communication between you, your insurance company and our practice is vital to the success of our relationship. VSA contracts with many insurance plans, and just as we have an obligation to abide by our contract with the insurance company, our patients also have contract responsibilities with their insurance plan. Below is a list of topics that will help you understand your financial obligations.

Insured Patients: For those patients who have insurance coverage, Vascular Surgery Associates will submit a claim on your behalf to the insurance companies with which we have a contract. Please be prepared to present all of your insurance cards at each visit. Please see our list of Participating Insurance Companies.

Preauthorization: Many insurance plans require us to notify them in advance of any procedure. Vascular Surgery Associates will make every attempt to get your procedure approved through your insurance carrier. However, the final decision is with your insurance company. Please note, your insurance company reminds us that “preauthorization of a procedure is not a guarantee that your insurance plan will pay for the service.”

Non-Covered Services: There may be times when your insurance company may deny coverage for a service. Any non-covered services will become your responsibility and will be considered a self-pay service. Questions regarding your specific coverage should be directed to your insurance company. You can find the number on the back of your card.

Self Pay: Patients who do not have insurance, or who would like to obtain services that are considered non-covered services by their insurance carrier, are considered self pay. Self Pay patients are responsible for payment in full at the time of services, unless prior arrangements have been made with one of our Patient Financial Liaisons. Referrals: Vascular Surgery Associates is a specialty practice. Some insurance plans require the patient to obtain a referral from their primary care physician. If your plan requires you to have a referral, you are responsible for obtaining the referral before scheduling an appointment with our group. Please check with your plan to see if you need a referral to a specialist.

Co-Payments: Co-Payments are determined by your insurance company. We are required by our contract with your insurance company to collect co-payments at the time of service. Please come to each visit prepared to pay your co-payment. If you are having ultrasound testing in our office, you may be required to pay an additional co-payment for this service. Please check with your insurance company prior to your visit to see if you have a separate co-payment for office testing. If you have Medicare and do not have secondary supplemental coverage, you are responsible for 20% of your visit at the time of service.

Deductibles: Deductibles are determined by your insurance company. If you have not met your deductible for the year, Vascular Surgery Associates will collect payment for the service at the time of the visit. Please check with your insurance company prior to your visit to see if you have met your deductible for the year. Deductibles are part of the premium you pay for your insurance coverage, therefore by law we cannot waive or discount deductibles.

Account Balances: Vascular Surgery Associates will provide you with a monthly statement of account activity in order to inform you when the balance becomes your responsibility as determined by your insurance carrier.

Outstanding Patient Balances: Any patient balance after your insurance company has made their determination is due and payable. Please come to each visit prepared to pay any outstanding balance or contact our Patient Financial Liaisons to make payment arrangements prior to your appointment. If payment is not received within 60 days of becoming patient responsibility, a finance charge, plus all costs of collection, including attorney fees, may be added to any outstanding balance.

Forms of Payment: Vascular Surgery Associates accepts cash, checks, money orders and major credit cards as forms of payment.
